The registered nurse shall visit the client in the client’s residence to assess the client’s general condition,
progress and response to services provided.
PERSONAL CARE SERVICES
Personal Care Services (PCS) covers the services of an aide in the recipient’s private residence to assist with the recipient’s
personal care needs such as, meal preparation, feeding, bathing, dressing, toileting, etc. that are directly linked to a medical
condition. PCS is a paraprofessional service and does not include skilled medical or skilled nursing care.
ACTIVITIES OF DAILY LIVING (ADLs) – DEFINITION
Basic personal care usually performed by an individual during the course of the day including ambulation, bathing, bed mobility, dressing,
eating, personal hygiene, toilet use, and transfers.
These activities are directly linked to the recipient’s medical condition or diagnosis. These activities are usually performed by
unlicensed paraprofessionals and do not constitute skilled medical or skilled nursing care.
PEDIACTRIC/ADOLESENT PERSONAL CARE SERVICES
Children with injuries or disabilities might have difficulties performing everyday tasks by themselves. With the help of our care team at

on achieving their independence and living the full life they have yet to experience.
SENIOR/DISABLED PERSONAL CARE SERVICES

If they neglect their personal care tasks, it might affect their overall health and well-being. To ensure that we fulfill our mission of serving our clients,
we will make sure that we provide proper assistance and respect to the elderly and persons with disabilities so that they can continue to live life with dignity.
COMPANION CARE/SITTER CARE
An elderly companion or senior sitter is a non-medical caregiver that can be there whenever and wherever clients need them whether that be full-time, part-time, 24/7.
Care can take place in a variety of settings such as senior living housing, nursing homes, hospitals, rehabilitation facilities, etc., but most often takes
place within a home setting. However, elderly sitters can only perform certain tasks that are not medically related or involve physical contact (such as ADLs),
so their roles are generally limited to providing basic supervision and companionship that can also include light housekeeping and errands depending upon the
agreement made with the caregiver.
RESPITE CARE
Respite Care is temporary care of a dependent elderly, ill, or handicapped person, providing relief for their usual caregivers.
PRIVATE PAY HOME CARE
Private pay home care is a service provided to elderly individuals who are in need of assistance with day-to-day activities.
A private pay home caregiver visits your home or the home of your loved one and helps with activities such as meal preparation,
housekeeping, and personal grooming and hygiene. This form of home care consists of hourly rate care packages.
CAP/C- (COMMUNITY ALTERNATIVES PROGRAM FOR CHILDREN)
CAP/C seeks to allow recipients who need long-term institutional care to return to or remain in the community.
It is designed to serve a limited number of medically fragile children who would be institutionalized in a nursing
facility or hospital if payment for home care services were not available through this NC Medicaid (Medicaid) CAP/C waiver.
CAP/C exists to supplement rather than replace the formal and informal services and support already available to a child.
CAP/C waiver services that Health-Pro provides are specified below.

CAP/DA- (COMMUNITY ALTERNATIVES PROGRAM FOR DISABLED ADULTS)
The Community Alternatives Program for Disabled Adults (CAP/DA) is a Medicaid Home and Community–Based Services (HCBS) Waiver Program. The CAP/DA program waives
certain NC Medicaid (Medicaid) requirements to furnish an array of home and community based services to adults who are elderly or disabled.
The services are designed to provide an alternative to institutionalization for recipients in this target population who prefer to remain in their homes,
and would be at risk of institutionalization without these services.
CAP/DA supplements rather than replaces the formal and informal services and supports already available to an individual. CAP/DA services are intended
for situations where no household member, relative, caregiver, landlord, community/volunteer agency, or third party payer is able or willing to meet the
needs of the recipient.

NURSING POOL SERVICES
A Nursing pool is any person, firm, corporation, partnership, or association engaged for hire in the business of providing or procuring temporary
employment in health care facilities for nursing personnel; including licensed nurses, nursing assistants, nurses’ aides, and orderlies. “Nursing pool”
does not include an individual who engages solely in providing his own services on a temporary basis to health care facilities.
PRIVATE DUTY NURSE
Private duty nurses provide a variety of services to support their clients' medical, therapeutic, and daily living needs.
Services often include: Skilled nursing care: This encompasses assessing vital signs, administering medications and injections, wound care, and monitoring medical equipment.
These services require a licensed individual, such as, a Registered Nurse or Licensed Practical Nurse.
