* Taking clients to appointments and providing social support

SKILLED NURSING

Skilled nursing care duties include: administering medications, monitoring vital signs, managing wounds, providing wound care, assisting with personal care needs, educating patients on self-care practices, coordinating with other healthcare providers, and overseeing overall health.

Key responsibilities:

* Medication management: Accurate administration of prescribed medications, including monitoring for side effects and coordinating refills.

* Vital signs monitoring: Regularly checking blood pressure, heart rate, temperature, and respiration.

* Cleaning and dressing changes for wounds, including pressure ulcers.

* Intravenous therapy: Managing IV infusions if necessary

* Ostomy care: Providing care for patients with ostomies

* Care planning: Developing and implementing individualized care plans based on patient needs

* Patient education: Teaching patients and caregivers about managing their health conditions, including medication usage and self-care techniques.
